The VF link state can be changed via the 'ip link set' cmd.
Currently, the new link state does not take effect immediately.

The fix is for the PF to send a link change async event to the
designated VF after a VF link state change.  This async event will
trigger the VF to update the link status.

After generating the random MAC address for VF, call the firmware to
approve it.  This step serves 2 purposes.  Some hypervisor (e.g. ESX)
wants to approve the MAC address.  2nd, the call will setup the
proper forwarding database in the internal switch.

We need to unlock the hwrm_cmd_lock mutex before calling bnxt_approve_mac().
We can do that because we are at the end of the function and all the
previous firmware response data has been copied.

The existing code is inconsistent in reporting and accepting the combined
channel count.  bnxt_get_channels() reports maximum combined as the
maximum rx count.  bnxt_set_channels() accepts combined count that
cannot be bigger than max rx or max tx.

For example, if max rx = 2 and max tx = 1, we report max supported
combined to be 2.  But if the user tries to set combined to 2, it will
fail because 2 is bigger than max tx which is 1.

Fix the code to be consistent.  Max allowed combined = max(max_rx, max_tx).
We will accept a combined channel count <= max(max_rx, max_tx).

Using Ethtool flashdev command, entire NVM package (*.pkg) files
may now be staged into the "update" area of the NVM and subsequently
verified and installed by the firmware using the newly introduced
command: NVM_INSTALL_UPDATE.

We also introduce use of the new firmware command FW_SET_TIME so that the
NVM-resident package installation log contains valid time-stamps.

The hardware is unable to drop rx packets not matching the RX filters.  To
workaround it, we create a special VNIC and configure the hardware to
direct all packets not matching the filters to it.  We then setup the
driver to drop packets received on this VNIC.

This patch creates the infrastructure for this VNIC, reserves a
completion ring, and rx rings.  Only shared completion ring mode is
supported.  The next 2 patches add a NAPI to handle packets from this
VNIC and the setup of the VNIC.

Nitro A0 has a hardware bug in the rx path.  The workaround is to create
a special COS context as a path for non-RSS (non-IP) packets.  Without this
workaround, the chip may stall when receiving RSS and non-RSS packets.

Add infrastructure to allow 2 contexts (RSS and CoS) per VNIC.  Allocate
and configure the CoS context for Nitro A0.

bnxt_en: Do function reset on the 1st PF open only.

Calling the firmware to do function reset on the PF will kill all the VFs.
To prevent that, we call function reset on the 1st PF open before any VF
can be activated.  On subsequent PF opens (with possibly some active VFs),
a bit has been set and we'll skip the function reset.  VF driver will
always do function reset on every open.  If there is an AER event, we will
always do function reset.

The port number for GENEVE is hard coded into the bnxt driver.  This is the
kind of thing we want to avoid going forward.  For now I will integrate
this back into the port notifier so that we can change the GENEVE port
number if we need to in the future.

This patch ends up doing several things.  First it updates the driver to
make use of the new unified UDP tunnel offload notifier functions.  In
addition I updated the code so that we can work around the bits that were
checking for if VXLAN was enabled since we are now using a notifier based
setup.

Samsung uses D2H command with Vendor Uniq Command (VUC) code F7
(the 0th bit of which is 1) for retrieving memory dump. In UEK4,
Bit 0 of the D2H command code has to be 0. Because of this voilation,
the nvmecli is unable to do crash and memory dumps in UEK4.

As the Samsung firmware can only understand VUC command code F7,
the IO direction is reversed for this vendor command code to
retrieve memory and crash dump.

Call i40e_client_get_params only after the instance is checked

We can avoid the minor bit of work by calling check params after we
check for the client instance, since we're about to return early in
cases where we do not have a client. But, more importantly, this
change prevents a panic from a NULL pointer when attempting to
get the client params.

vxlan: do not receive IPv4 packets on IPv6 socket

By default (subject to the sysctl settings), IPv6 sockets listen also for
IPv4 traffic. Vxlan is not prepared for that and expects IPv6 header in
packets received through an IPv6 socket.

In addition, it's currently not possible to have both IPv4 and IPv6 vxlan
tunnel on the same port (unless bindv6only sysctl is enabled), as it's not
possible to create and bind both IPv4 and IPv6 vxlan interfaces and there's
no way to specify both IPv4 and IPv6 remote/group IP addresses.

Set IPV6_V6ONLY on vxlan sockets to fix both of these issues. This is not
done globally in udp_tunnel, as l2tp and tipc seems to work okay when
receiving IPv4 packets on IPv6 socket and people may rely on this behavior.
The other tunnels (geneve and fou) do not support IPv6.

When we are in connection recovery and the internal command timer on a
request pops, either the scsi_cmnd->device or scsi_cmnd->device->host
back pointers may be NULL as the device that the command that the
request was submitted on may have been subsequently reaped due to the
connection recovery. This can cause one or both of the pointers above to
be NULL and cause a system crash if we try to return the command to the
midlayer.

Instead, double check the pointers before the return to the midlayer so
as to prevent the crash and let the upper layers finish the session
recovery and rediscover the device.

Oracle discovered that the NVMe driver gets SQ completion errors eventually
leading to the device being reset, taken out of the PCI bus tree or kernel
panics when using the default SQ size of 1024 entries (64KB) for Samsung
EPIC NVMe SSDs.

PCIe analyzer tracing by Oracle and Samsung revealed an errata in Samsung's
firmware for EPIC SSDs where these invalid completion entries can occur
when the queues straddle an 8MB DMA address boundary.

This patch works around the errata by detecting these specific devices and
limiting their descriptor queue depth to 64.  This is only for the Samsung
NVMe controllers used in Oracle X-series servers.

There was no noticeable performance impact of reducing queue depths to 64
for these Samsung drives, Oracle X6-2 server, and Oracle VM Server 3.4.2.

When "primary_reselect" is set to "failure", primary interface should
not become active until current active slave is down. But if we set first
member of bond device as a "primary" interface and "primary_reselect"
is set to "failure" then whenever primary interface's link gets back(up)
it becomes active slave even if current active slave is still up.

With this patch, "bond_find_best_slave" will not traverse members if
primary interface is not candidate for failover/reselection and current
active slave is still up.

processes are sent SIGTERM.

The current NVME driver allocates I/O queue per-cpu and alloctes IRQ
per-queue for the devices, a large number of IRQs will be allotted to
the I/O queues on a large NUMA/SMP system with multiple NVME devices
installed because of this design.

It would cause failure of CPU hotplug operations on the above mentioned
system, the problem is that the CPU cores could not be hotplugged after
certain number of them are offlined because the remaining online CPU cores
have not enough IRQ vectors to accept the large number of migrated IRQs.

This patch fixes it by providing a way to reduce the NVME queue IRQs to
an acceptable number.

nvme: Remove RCU namespace protection

We can't sleep with RCU read lock held, but we need to do potentially
blocking stuff to namespace queues when iterating the list. This patch
removes the RCU locking and holds a mutex instead.

To prevent deadlocks, this patch removes holding the mutex during
namespace scanning and removal. The unlocked namespace scanning is made
safe by holding a reference to the namespace being scanned.

List iteration that does IO has to be unlocked to allow error recovery.
The caller must ensure the list can not be manipulated during such an
event, so this patch adds a comment explaining this requirement to the
only function that iterates an unlocked list. All callers currently
meet this requirement, so no further changes required.

List iterations that do not do IO can safely use the lock since it couldn't
block recovery from missing forced IO completions.

state is only valid in the path from scsi_remove_target() to
scsi_target_destroy() indicating this target is going to be removed.

This re-fixes the problem introduced in commits bc3f02a795d3 ("[SCSI]
scsi_remove_target: fix softlockup regression on hot remove") and
40998193560d ("scsi: restart list search after unlock in
scsi_remove_target") in a more comprehensive way.

What appears to be happening is that something has pinned the target
so it can't go into STARGET_DEL via final release and the loop in
scsi_remove_target spins endlessly until that happens.

The fix for this soft lockup is to not keep looping over a device that
we've called remove on but which hasn't gone into DEL state.  This
patch will retain a simplistic memory of the last target and not keep
looping over it.

This patch validates the num_values parameter from userland during the

megaraid_sas: Don't issue kill adapter for MFI controllers in case of PD list DCMD failure

There are few MFI adapters which do not support MR_DCMD_PD_LIST_QUERY so
if MFI adapters fail this DCMD, it should not be considered as FATAL and
driver should not issue kill adapter and set per controller's instance
variable- pd_list_not_supported so that same variable can be used inside
functions- slave_alloc and slave_configure to allow firmware scan.

Killing adapter because of DCMD failure when this DCMD is not supported
causes driver's probe getting failed. This issue got introduced by
commit 6d40afbc7d13 ("megaraid_sas: MFI IO timeout handling").

Killing adapter in case of this DCMD failure should be limited to Fusion
adapters only. Per controller's instance variable allow_fw_scan is
removed as pd_list_not_supported better reflect the purpose.

Earlier versions of the mptsas driver included a mechanism for
executing, and if necessary retrying, SCSI TEST UNIT READY commands
to ensure that devices complete their initialization during device
discovery.  This functionality, present in UEK2, was never sent
upstream, and was lost when UEK4 was initiated.

We have been seeing flash devices returning errors, or simply
disappearing, during alter cell validate configuration operations
on Exadata systems.  Giving the flash disks time to initialize
after (re-) discovery appears to resolve this issue.

This commit simply restores the missing functionality.

This "backports" the structure I've used for the fabrics driver.  It
mostly started out as a cleanup so that I could actually understand
the code, but I think it also qualifies as a micro-optimization due
to the reduced time we hold q_lock and disable interrupts.

qede: Add support for handling IP fragmented packets.

When handling IP fragmented packets with csum in their
transport header, the csum isn't changed as part of the
fragmentation. As a result, the packet containing the
transport headers would have the correct csum of the original
packet, but one that mismatches the actual packet that
passes on the wire. As a result, on receive path HW would
give an indication that the packet has incorrect csum,
which would cause qede to discard the incoming packet.

Since HW also delivers a notification of IP fragments,
change driver behavior to pass such incoming packets
to stack and let it make the decision whether it needs
to be dropped.

In the current code "ent_per_page" could be more than "conn_num" making
"conn_num" negative after the subtraction.  In the next iteration
through the loop then the negative is treated as a very high positive
meaning we don't put a limit on "ent_num".  It could lead to memory
corruption.

If a future VF would send the PF an unknown message, the PF today would
not send a reply. This would have 2 bad effects:
  a. VF would have to timeout on the request.
  b. If VF were to send an additional message to PF, firmware would mark
     it as malicious.

Instead, if there's some valid reply-address on the message - let the PF
answer and tell the VF it doesn't know the message.
